Seniors' Community Connector - Social Prescribing Program - Fort St. John
Seniors Services
Provided by North Peace Seniors Housing Society
Eligibility: Any older adult 60+ residing in Fort St. John and surrounding areas who is either experiencing or is at risk of:
- Frailty
- Social Isolation and/or Loneliness
- Barriers to Accessing Community-Based Resources
- Frequent Trips to Emergency Department or Primary Care
- Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of-Life, or Palliative Care
Referrals accepted by email or phone call (self referrals and referral by family member are fine).
